National Institute for Health and Care Excellence (NICE) har utf\u00e4rdat riktlinjer som rekommenderar anv\u00e4ndning av Ex Vivo Lung Perfusion (EVLP) vid preservation av lungor. EVLP st\u00f6ds som standardf\u00f6rfarande, vilket \u00e4r den mest positiva rekommendationen som NICE kan utf\u00e4rda.

NICE-riktlinjer utg\u00f6r evidensbaserade rekommendationer vilka ska anv\u00e4ndas som beslutsv\u00e4gledning f\u00f6r fr\u00e5gor inom h\u00e4lsa, folkh\u00e4lsa och omv\u00e5rdnad i Storbritannien och Wales. NICE utv\u00e4rderar s\u00e4kerheten och effektiviteten hos interventionella procedurer.

Idag anv\u00e4nds endast en av fem donerade lungor f\u00f6r transplantation i Storbritannien. Bristen p\u00e5 avlidna donatorlungor leder till \u00f6kad sjuklighet och d\u00f6d f\u00f6r upp till 30% av de patienter som v\u00e4ntar p\u00e5 en lungtransplantation. Det finns f\u00f6r n\u00e4rvarande 257 patienter som v\u00e4ntar p\u00e5 en lungtransplantation i Storbritannien och den genomsnittliga v\u00e4ntetiden \u00e4r hela 18 m\u00e5nader.

I den nyligen publicerade NICE-rekommendationen beskrivs EVLP som en teknik som m\u00f6jligg\u00f6r l\u00e4ngre och b\u00e4ttre preservation av donerade lungor. Detta kan m\u00f6jligg\u00f6ra att donerade lungor med marginell funktion som tidigare inte kunnat anv\u00e4ndas f\u00f6r transplantation nu kan utv\u00e4rderas och transplanteras.

Hassiba Smail, \u00f6verl\u00e4kare inom thoraxkirurgi vid Harefield Hospital, London, kommenterar: \u201dV\u00e5ra patienter p\u00e5 v\u00e4ntelistan v\u00e4ntar desperat p\u00e5 chansen till att erh\u00e5lla en lungtransplantation och n\u00e5gra av dem f\u00e5r tyv\u00e4rr inte denna m\u00f6jlighet. De perfekta donatorlungorna \u00e4r s\u00e4llsynta, lungor med marginell funktion \u00e4r de vanligaste. F\u00f6r kirurger \u00e4r det idag en stor utmaning att med trygghet kunna acceptera och transplantera dessa marginella lungor. EVLP har visat sig vara det verktyg som hj\u00e4lper oss kirurger att fatta detta beslut.\u201d
